Dear colleagues, please fill in this questionnaire so that you can be assigned an appropriate date for testing for the presence of SARS-CoV-2 antigen.

The place where all students will be tested is the amphitheater of the Clinic for Neurosurgery of the Clinical Center of Serbia.

 

The questionnaire is filled in exclusively by 4th and 5th year students who

1) have not been vaccinated,

2) have been vaccinated with only one of 2 doses of vaccine and

3) those who have not had more than 2 weeks since the second dose of vaccination.

 

Completing the questionnaire is mandatory!

From the information, it is necessary to enter the email address, name and surname, index number, year of study and date of testing.

 

Testing dates are Monday, Tuesday and Wednesday. Students choose the term based on the schedule of practical classes:

 

• Students who are scheduled for the first practicals during the week on Monday can choose some of the scheduled appointment for testing on Monday.
• Students who are scheduled for the first practicals during the week on Tuesday can choose some of the scheduled appointment for testing on Tuesday.
• Students who are scheduled for the first practicals during the week on Wednesday choose some scheduled appointment for testing on Wednesday.
